Primary Home Care Program (PHC)
Primary Home Care is a nontechnical , medically related personal care
services prescribed by a physician as part of a client's plan of care.
Primary Home Care is available to eligible Medicaid clients whose health
problems cause them to be functionally limited in performing activities
of daily living. Each client's services are provided by a primary home
care attendant.
Personal care tasks include:
- Bathing/Grooming/Skin Care
- Feeding/Eating/Meal Preparation
- Exercise/Walking/Transfers
Home management tasks include:
- Cleaning
- Laundering
- Grocery Shopping
Escort:
The attendant will accompany the client outside the home to support
the client with living in the community to necessary medical appointments
and grocery shopping.

First Care Home Health Services has a contract with the Department of
Aging and Disability Services (DADS) to provide services in your area.
